The APOE4 Lipid Problem Is Nuanced
Cholesterol is essential for brain health. Our brains depend on cholesterol and healthy fats for:
The issue in APOE4 is not simply “high cholesterol.” The issue is:
- Synapse formation
- Neuronal repair
- Myelin integrity
- Hormone production
- Cellular signaling
- Energy metabolism
- How lipids are transported
- How they are oxidized
- How efficiently they are cleared
- Whether the brain is receiving adequate fuel
For this reason, I do not pursue extreme lipid lowering.
The goal is balance:
- Reduce vascular injury
- Lower atherogenic particle burden
- Minimize oxidation and inflammation
- Preserve adequate brain fuel and hormone production
Why ApoB Matters
ApoB reflects the number of potentially damaging lipoprotein particles.
This is often more informative than LDL alone.
Many APOE4 carriers can present with:
- Reasonable LDL levels
- But excessive particle burden
- Increased oxidation
- Persistent vascular inflammation
I focus more on:
- Particle quality
- Inflammatory burden
- Oxidation status
- Metabolic health
…rather than LDL alone.

Sterol Absorption
Particularly important in APOE4 carriers.
I pay close attention to:
- Beta-sitosterol
- Campesterol
- Lathosterol
- Desmosterol
Some APOE4 carriers are sterol hyperabsorbers and may respond poorly to excessive plant sterol intake despite otherwise healthy diets.
My Medication Strategy
Ezetimibe
Particularly useful for sterol hyperabsorbers. And big bonus: Shown to reduce risk of AD.
May help reduce:
- ApoB
- LDL-C
- Plant sterol absorption
Especially helpful when absorption markers are elevated.
Pravastatin
A hydrophilic statin often considered less disruptive from a cognitive and mitochondrial standpoint compared with some lipophilic statins.
In some individuals, low-dose or intermittent dosing may provide benefit while minimizing side effects.

- Exercise
- Muscle mass
- Sleep
- Inflammation
- Liver function
- Stress biology
Lipid optimization is only one component of brain protection.

I do not view cholesterol as “the enemy.”
For APOE4 carriers, the goal is:
- Better lipid trafficking
- Reduced vascular injury
- Lower inflammation
- Lower oxidation
- Stable metabolic function
- Adequate brain fuel delivery
The objective is long-term brain resilience – not simply driving cholesterol as low as possible.
